Mosaic Health has been working for six decades to provide greater access to vital health care services to underserved patients of all ages, ethnicities, and backgrounds. That mission has only expanded as we’ve opened health centers throughout New York State, from the Finger Lakes Region to the Mohawk Velley.
Through the years, Mosaic Health has partnered with dozens of other organizations in New York to address issues ranging from lack of access to care for people of color, lead poisoning in children, HIV treatment and prevention, and exercise for seniors. Through it all, our mission, our vision, and our values hold true.
How we started, why we’re here, and what we stand for.
Our History
1964:
Founded in Rochester, New York, as Neighborhood Health Centers of Monroe County (NHCMC).
Early 1980s:
Became known as Rochester Health Network (RHN) and founded a federally compliant grant board to oversee the community health care grant program.
1987:
Rochester Primary Care Network (RPCN) launched to comply with government requirement for a separate corporation for the community health care grant program.
2010:
Expanded beyond the city of Rochester, stretching south through the Finger Lakes and east to of Utica.
2016:
Expanded further east with the opening of Valley Family Health Center in Ilion.
2019:
Renamed Mosaic Health, reflecting the wonderfully diverse communities we serve.
Mission
To provide access to compassionate, individualized health care and wellness-related education regardless of financial, cultural, or social barriers.
Vision
To be the healthcare provider of choice in upstate New York and to create healthy communities in which health disparities are diminished and there is access to quality care.
