What’s an FQHC? What about a PCMH?
These four-letter abbreviations described to of the most important parts of what we do. Here’s what they mean.
FQHC is short for Federally Qualified Health Center.
This designation for certain health centers was created to describe how we provide accessible, continuous, coordinated, and comprehensive patient-centered care, from family medicine to dental services to mental health and substance abuse. Our health centers are FQHCs, which means we’re here to serve the most vulnerable, underserved and uninsured populations. Our work strengthens communities by improving public health.
FQHCs are different from some health organization because we offer:
- Case management
- Patient education
- Translation/interpretation
- Community education
FQHCs also:
- Provide needed services such as free immunizations for uninsured children
- Act as a community voice for health care needs through the Board of Directors
- Help patients afford health care regardless of insurance status
- Guide patients through enrollment in Medicaid, CHIP, and other assistance programs
PCMH is short for Patient-Centered Medical Home.
This program is designed to improve primary care and strengthen the patient-provider relationship. A certified PCMH follows standards for patient care, coordinated teamwork, and tracking and coordinating care over time.
Our Ilion, Rushville, and Utica health centers are each certified as a Level 3 PCMH. Each one fosters long-term healing relationships between patients and providers and care coordinators. Patients get the care they need when they need it, in the language they speak, and in a culturally appropriate manner.
Why a PCMH is good for patients.
At a PCMH-certified facility, you get personalized, effective care because you have a more personal relationship with your provider—plus you have a whole team assigned to your care.
