Mosaic Health makes it easy for you to pay for your health care visits.
We welcome all private pay, fully insured, partially insured, Medicare, and Medicaid patients. If you have insurance, you must present your current insurance card at the time of your visit. And for patients without insurance, we offer a sliding fee scale based on family size and household income to ensure access to quality, compassionate health care.
Co-pays and self-payments are due at the time of service. We accept cash, checks, and credit card payments.
Our sliding fee scale makes sure you can still get the health care services you need, regardless of income or insurance status. This program offers discounts on services based on family size and household income. Patients approved for the Sliding Fee Discount Program must also pay their co-pay at the time of the visit.
Uninsured? Please call the Mosaic Health office near you and we can see what coverage you qualify for in New York State and help you enroll.
No one will ever be turned away. We accept all patients regardless of your ability to pay.
