Mosaic Health makes it easy for you to pay for your health care visits.

We welcome all private pay, fully insured, partially insured, Medicare, and Medicaid patients. If you have insurance, you must present your current insurance card at the time of your visit. If you don’t have insurance, we offer a sliding fee scale based on family size and household income. The bottom line: No one will ever be turned away. We accept all patients regardless of your ability to pay.

Forms of payment

Co-pays and self-payments are due at the time of service. We accept cash, checks, and credit card payments.

Our sliding fee scale

The sliding fee scale program makes sure you can still get the health care services you need, regardless of income or insurance status. This program offers discounts on services based on family size and household income. Patients approved for the Sliding Fee Discount Program must also pay their co-pay at the time of the visit.

If you have health insurance

We accept most insurance. If you are a new patient or your coverage has changed, please call your Mosaic Health office and your insurance carrier to see whether we accept your insurance.
