Mosaic Health Utica is located in the Mohawk Valley in Utica, New York.  The center was opened by Mosaic Health (formerly RPCN) and its partners in 2010, to improve access to medical and dental care in the city after it was identified by HRSA as being a high need area. At the time, it was the only Federally Qualified Health Center in Oneida County.

Oneida street is now home to medical and behavioral health services and was recently renovated to accommodate an increase in patient needs. Dental services have moved to 3 Parkside Court as part of this expansion project to increase capacity in the community. Utica Dental offers eight operatories, brand new chairs, and state-of-the art equipment. A social work care manager is on site to help address social needs, including financial assistance. The site is also the new home for the Mohawk Valley Community Dentistry team.

All Utica patients are managed by a care team, which includes the providers you see at the center—such as your physician, physician assistant and nurse practitioners, dentist, hygienist, dental assistant, social workers, and care managers—and are referred to local providers and agencies for non-clinical support services.

The care team will make sure you receive accurate and timely treatment and can be a source of guidance, support, education, and empowerment. They coordinate appointments and services and build strong relationships with other health care team members by keeping communication lines open with each other and the patient. They will also help you:

  • Offer support, guidance, and instruction

  • Learn about the health care system

  • Understand your diagnosis and treatment

  • Find available educational resources and supportive services

  • Get answers to insurance and financial concerns by connecting you to appropriate resources

  • Coordinate appointments and transportation assistance

  • Provide the answers you need and help you understand the information delivered

  • Make referrals to appropriate services such as behavioral health, nutrition, case management, medication assistance, and financial counseling

All forms of health insurance are welcome, including Medicaid and Medicare. Utica also accepts patients without insurance or with limited insurance. Sliding Fee discounts are available to those who qualify.

Medical patients are eligible for behavioral health services. Ask your provider for more information.

Interpreter services are available.

Diabetes Program

Utica offers educational programs for its diabetic and pre-diabetic patients to improve disease management and reduce the number of complications. Topics include:

  • Nutrition

  • Physical Activity

  • Heart Disease

  • Stroke
