Valley Family Health Center Recognized as Patient Centered Medical Home
ILION, NY – February 5, 2018 – Valley Family Health Center, the newest Center opened by Regional Primary Care Network (RPCN) in Ilion in the spring of 2016, earned national recognition as a Patient Centered Medical Home in December, 2017.
Like the rest of RPCN’s network of health centers, the National Committee for Quality Assurance (NCQA) has recognized Valley Family Health Center (VFHC) as a Level 3 Patient Centered Medical Home— the top award for delivery of comprehensive, high-quality care and service. The PCMH care delivery model strengthens the patient-physician relationship by focusing on preventive care rather than reactive care. And the designation is earned when practices use evidence-based, patient-centered processes that focus on highly-coordinated care and long-term relationships between clinicians and patients.
